Was Forest Surgicanter at fault because of bad equipment? Did a bad cannula break? Did anesthesia play a roll by allowing the patient to wake up and jump around. Was Bay Area Surgical Management at fault for hiring unlicensed surgical techs or for not having adequate resuscitation plans and equipment? a |

If the cutting of the vein was due to the surgeon being intoxicated (for example) The waiver would mean nothing. |
